Top Notch Elite Volleyball Liability Waiver & Release of Claims Policy
I. ACKNOWLEDGMENT OF INHERENT RISKS
Participants and parents/guardians recognize that volleyball activities involve unavoidable risks, including but not limited to:
Bodily injury (e.g., sprains, fractures, concussions)
Illness (e.g., communicable diseases like COVID-19 or influenza)
Permanent disability or death
Property damage
II. RELEASE OF LIABILITY
In exchange for participation, the undersigned irrevocably releases and discharges Top Notch Elite Volleyball, its directors, coaches, staff, volunteers, and facilities ("Released Parties") from:

All claims for injury, illness, disability, death, or property damage

Claims arising from negligence of the Released Parties

Expenses (e.g., medical costs, lost wages)
III. ASSUMPTION OF RISK
The undersigned voluntarily assumes all known and unknown risks associated with volleyball activities, including travel and events.
IV. INDEMNIFICATION AGREEMENT
Participants and parents/guardians agree to:

Defend and indemnify Released Parties against third-party claims

Cover related legal fees and costs
V. PARENT/GUARDIAN OBLIGATIONS
For minor participants, parents/guardians:

Explain activity risks and rules to the minor

Confirm the minor understands this agreement

Waive all claims on behalf of the minor and family
VI. LEGAL COMPLIANCE
This waiver meets Oklahoma enforceability standards by:

Clearly identifying Released Parties (Top Notch Elite Volleyball, staff, agents)

Specifying waived acts (negligence in volleyball activities)

Describing covered damages (injury, illness, death, property loss)

Being signed voluntarily without duress
Policy Administration

Signed waivers retained for 7 years post-participation

Non-compliance voids participation eligibility
