I, the undersigned parent or legal guardian hereby grant permission to Top Notch Volleyball Club of BA and its representatives to photograph and/or record video of my child during volleyball-related activities. I authorize Top Notch Volleyball Club of BA to use these photographs and/or video recordings of my child for any lawful purpose, including but not limited to publicity, illustration, advertising, and web content.

I understand that these images and recordings may be used in various media formats, including print publications, online publications, websites, and social media platforms. I waive any right to inspect or approve the finished product wherein my child's likeness appears.

I acknowledge that neither I nor my child will receive financial compensation for the use of these photographs and/or video recordings. I agree that Top Notch Volleyball Club of BA owns the copyright to these materials and I hereby release Top Notch Volleyball Club of BA and its representatives from any claims that may arise regarding the use of my child's image or voice.

I understand that this release is valid for an indefinite period and that I may not revoke this consent once given. However, I may request in writing that Top Notch Volleyball Club of BA cease using my child's image in future materials.

By signing below, I confirm that I am the parent or legal guardian of the child named above, I have read and understand the contents of this release, and I consent to its terms on behalf of my child.
